Please submit one survey per person.  To clarify, this is just an informal survey to get community feedback about the Fall leagues not counting toward NTRP Ratings.  This is not a vote to change the league parameters; however, the Leagues Committee would like this feedback to make informed decisions about future parameters. All fields are required.  Surveys submitted without USTA numbers will not be accepted nor will duplicate or fraudulent submissions.
